During a gynecological exam, the doctor does an internal exam with the speculum in at first, just looking & taking swabs, THEN the speculum is removed before fingers are inserted to check positioning of the cervix/uterus/ovaries & for lumps, etc. The digital exam NEVER done with the speculum still inserted. I found this detail incredibly distracting to the point where I couldn't finish the story.
