Why can't this jackass just admit he loves Oliver and treat him decent in public?

It's like he's painfully afraid of intimacy. His marriage is obviously a sham and Oliver is the only person he seems to care for outside of sex. What next does he have to almost die before Alex will admit to his feelings?
