Ouch. My nipples are only about a centimeter long - having them stretched to the point of multiple inches long would mean ripped, smashed, or otherwise torn off. That killed the story's pleasure for me. Distended or elongated under the pressure would've been an apt description, but unless she has long nipples even prior to childbirth, multiple inches would be insanely painful and stretched, very sadistic. Not at all the mood that the rest of your story entails. Beyond the insane nipple-squishing, it was enjoyable, erotic without the sadism and humiliation threads so prominent in this genre.
