Having your scrotum filled with saline solution does not hurt. The inital sting of the needle being put in is just not that bad. Having the water fill the scrotum is a little weird the first time you do it. I have done it a number of times and really enjoy the feeling of having my sack full of water. I have done two liters at once and I will say here that it was a little bit over the top. But having my scrotum slapped and hit with a fist or even a empty wine bottle which is a solid and heavy item, feels good to me. I talked to a good urologist about doing this act, and his response was just make sure to be sterile at all times when putting the needle in and when you take it out.
